Switching to transdiagnostic treatment of worry and rumination following poor early response to CBT-E

چکیده
We describe the treatment of a patient with BN (plus comorbid major depression and generalised anxiety disorder) who completed stage one and stage two of CBTE at a specialist eating disorders outpatient clinic. Despite good compliance with all components of the treatment, the patient did not achieve an early rapid reduction in binge-eating and purging by the stage two progress review and was considered to be at-risk of suboptimal treatment outcome. Her repetitive negative thinking (worry and rumination) was identified as significantly interfering with progress, and the primary trigger for episodes of binge-eating and vomiting. In light of this, it was decided to evaluate the usefulness of abandoning CBT-E and switching to metacognitive therapy (MCT) for repetitive negative thinking (RNT). RNT is defined as cognitive perseveration on negative themes. MCT has been shown to demonstrate excellent outcomes in patients with anxiety and depressive disorders at the same clinic. Switching to a transdiagnostic metacognitive was associated with achieving optimal outcomes at the end of treatment. It is recommended that clinicians routinely assess the degree to which patients have achieved an early response to CBT-E and systematically evaluate the effectiveness of switching or pursuing CBT-E in this patient group. This abstract was presented in the Treatment in Community and Inpatient Settings stream of the 2014 ANZAED Conference.

متن کاملInvestigating Cognitive Control and Repetitive Thinking in Clinical Groups With Major Depressive Disorder and Generalized Anxiety Disorder
Objective: Some theories state that a deficiency in cognitive control makes people more vulnerable to the occurrence of repetitive negative thoughts. The present study was done to investigate the association between cognitive control based on switching and inhibition and repetitive thinking of rumination and worry and major depressive disorder and generalized anxiety disorder. Methods: Thirty ...
متن کاملTransdiagnostic emotion regulation processes in bipolar disorder and insomnia.
Research and treatment have traditionally adopted a 'disorder-focused' approach by targeting one specific disorder, aiming to understanding its cause, maintenance and treatment. The aim of the present study was to contribute to the burgeoning interest in examining common, or 'transdiagnostic,' processes across disorders. Three candidate transdiagnostic processes involved in emotion regulation -...
متن کاملO4: Trans-Diagnostic CBT and Comorbidity in the Anxiety Disorders
Recent research from a number of clinical trials has shown that Transdiagnostic Cognitive-Behavioral Therapy is efficacious, having shown comparable effects on primary anxiety disorder diagnoses as traditional diagnosis-specific CBT, and highly economical for treatment providers. However, a majority of anxiety disorder patients present with two or more “comorbid” diagnoses at once. Given that T...
متن کاملRelationship between trait repetitive negative thinking styles and symptoms of psychopathology
Anticipatory processing (AP) is a repetitive negative thinking (RNT) style associated with social anxiety that has been excluded in studies of repetitive thought (e.g., worry and rumination). The following studies examined whether individual differences in AP were associated with social anxiety symptoms above and beyond worry and rumination in undergraduate samples. Study 1 (N = 326) examined t...
